Billing
We accept cash, PayPal, Visa and Mastercard, or we can bill your insurance company. Charges not covered by your insurance such as deductibles, copays and balances are your responsibility. Payment is due at the time of service.

Cancellation Policy
As a courtesy to other patients, we ask that you give 24-hour notice for cancellations.
We strictly adhere to charging for missed appointments with less than 24-hour notice. We reserve the right to charge for the missed appointment at the rate of a normal office visit, which is approximately $100. This fee cannot be billed to your insurance company.
In the event that you are unable to provide sufficient notice of cancellation as outlined above, we will require your pre-authorized credit card information for any subsequent appointment scheduling. A subsequent missed appointment will result in a charge of the respective amount to your credit card.
